Import roster from CSV

Upserts on email. Expected columns: name, email, coach, payment_system, plan_term, term_months, amount, interval, start_date, renewal_date, status, notes. Coach values Jordan / Tannen assign that coach; Not eligible flags the member as an upsell target; blank leaves them unassigned. renewal_date of ongoing stores as no renewal date.
